Standard Zepbound dose schedule
Zepbound is titrated up gradually so your body can adjust to the medication and minimize gastrointestinal side effects. Most people stay at each dose for at least four weeks before stepping up. Your prescriber decides how high to go based on your tolerance and progress.
- 2.5 mgWeeks 1–4Starting dose. Not intended for weight loss; gets your body acclimated.

The 7.5 mg and 12.5 mg steps are optional bridges between maintenance doses. Most people land on 5, 10, or 15 mg long-term.

SURMOUNT-1 was a 72-week randomized trial of 2,539 adults with obesity (or overweight plus a weight-related condition) without diabetes. Participants were assigned to placebo or to 5, 10, or 15 mg of tirzepatide once weekly, alongside lifestyle counseling.

GI side effects are most common in the first weeks after starting and after each dose increase, then typically ease within a few weeks at a stable dose. Eating smaller, lower-fat meals and staying hydrated tends to help. Severe or persistent vomiting is a reason to contact your prescriber rather than push through.
