Standard Semaglutide dose schedule
Semaglutide is generally titrated up over about 16 weeks regardless of formulation, with dose targets that depend on whether the goal is glycemic control or weight management. Compounded protocols often follow the FDA brand schedules but can vary by prescribing pharmacy.
- 0.25 mgWeeks 1–4Standard starting dose across formulations.
- 0.5 mgWeeks 5–8
- 1.0 mgWeeks 9–12Common diabetes maintenance dose.
- 1.7 mgWeeks 13–16Wegovy intermediate dose.
- 2.0 mgOptionalOzempic maximum for diabetes.
- 2.4 mgMaintenanceWegovy weight-management maximum.
Compounded semaglutide may be prescribed at non-standard doses (e.g. 0.3 mg, 1.5 mg). Follow your prescriber’s exact protocol — compounded products are not FDA-evaluated for safety or efficacy at those doses.
What the trials show for Semaglutide
14.9% mean body-weight loss at 2.4 mg over 68 weeks (STEP-1)
The STEP-1 trial established semaglutide 2.4 mg weekly as a safe, effective therapy for chronic weight management. Compounded semaglutide is not separately studied at the same scale; outcomes are extrapolated from branded trials with appropriate caveats.

Semaglutide questions
Made for GLP-1 users,
by a GLP-1 user
After months on GLP-1s, I was frustrated. I found myself navigating forgotten doses, inconsistent progress, and juggling multiple tracking apps without getting much in return. So, I decided to build something better.
Over the last year, I built Sage to be the ultimate companion for GLP-1 medication. I prioritized bringing ease, clarity, and efficacy to tracking. And it's only going to get better.
