Standard Ozempic dose schedule
Ozempic uses a slower, lower-ceiling titration than Wegovy. The goal is to reach the dose that controls your blood sugar with the fewest side effects, not necessarily the maximum.
- 0.25 mgWeeks 1–4Starting dose. Not intended to lower blood sugar.
- 0.5 mgWeek 5+First therapeutic dose for diabetes.
- 1.0 mgOptional step-upAfter at least 4 weeks at 0.5 mg if more A1C reduction is needed.

Most people for diabetes settle at 0.5 or 1.0 mg long-term; 2.0 mg is reserved for those needing more A1C reduction.
What the trials show for Ozempic
26% reduction in major adverse cardiovascular events vs. placebo
SUSTAIN-6 was a 104-week cardiovascular outcomes trial of 3,297 adults with type 2 diabetes at high cardiovascular risk. Participants were randomized to weekly semaglutide (0.5 or 1 mg) or placebo on top of standard care.

GI side effects are most common in the first weeks and after each dose increase, and they usually ease as your body adapts. Hypoglycemia can occur when Ozempic is combined with insulin or sulfonylureas — talk to your provider about adjusting those medications when starting or stepping up.
