Standard Tirzepatide dose schedule
Tirzepatide titration follows the same step ladder regardless of brand or compounded formulation. Each step is held for at least four weeks before increasing, both to manage GI side effects and to assess your response.
- 2.5 mgWeeks 1–4Starting dose. Not intended for weight loss; gets your body acclimated.
- 5 mgWeeks 5–8
- 7.5 mgWeeks 9–12 (optional bridge)
- 10 mgWeeks 13–16
- 12.5 mgWeeks 17–20 (optional bridge)
- 15 mgMaintenanceMaximum approved dose.
Compounded tirzepatide may be supplied at non-standard concentrations or doses. Always follow your prescriber’s exact dosing instructions; compounded products are not FDA-reviewed for safety or efficacy at those doses.
What the trials show for Tirzepatide
22.5% mean weight loss at 15 mg over 72 weeks (SURMOUNT-1)
The SURMOUNT trials established tirzepatide for chronic weight management; the SURPASS program established it for type 2 diabetes. Both showed substantial efficacy at the 5, 10, and 15 mg weekly doses. Compounded tirzepatide is not separately studied at the same scale; outcomes are extrapolated from branded trials with appropriate caveats.

Common side effects of Tirzepatide
Tirzepatide-specific tolerability profile.
- Nausea
- Diarrhea
- Decreased appetite
- Vomiting
- Constipation
- Indigestion
- Abdominal pain
- Fatigue
Tirzepatide’s GI profile is similar to semaglutide’s, with side effects most prominent in the first weeks after each dose increase. Compounded formulations may include adjunctive ingredients with their own profiles. Severe vomiting, persistent abdominal pain, or signs of pancreatitis warrant calling your provider.

Tirzepatide questions
Made for GLP-1 users,
by a GLP-1 user
After months on GLP-1s, I was frustrated. I found myself navigating forgotten doses, inconsistent progress, and juggling multiple tracking apps without getting much in return. So, I decided to build something better.
Over the last year, I built Sage to be the ultimate companion for GLP-1 medication. I prioritized bringing ease, clarity, and efficacy to tracking. And it's only going to get better.
